What is HOPE at home?

It is a comfort oriented care offering a palliative approach for those with a life-threatening, life limiting or advanced illness or who may be receiving curative treatments.

What is Palliative-Supportive care?

To palliate means to make comfortable; treatments or procedures are provided when they will contribute to the comfort of the patient. The goal is to maintain the dignity of life while addressing physical, emotional, social and spiritual needs.

The HOPE at Home team works closely with the patient’s primary physician to develop a comprehensive plan of care for physical and emotional support.

How to arrange HOPE at Home

A physician must approve admission to the program. Early referrals are encouraged so that the patient and family can receive the full benefi ts of the program. Services are covered by Medicare, Medicade and most private insurance plans. For more information, please call us.
