New Milford Visiting Nurse Association Ranked Among Top 500 of Home Health Providers in the Country

For the third year in a row New Milford Visiting Nurse Association has been named to the national HomeCare Elite, a compilation of the most successful Medicare‐certified home health care providers in the United States. This annual review identifies the top 500 of agencies, ranked by an analysis of performance measures in quality outcomes, quality improvement and financial performance.

New Milford VNA is a nonprofit health care agency dedicated to providing quality home care, hospice and public health services to the greater New Milford area. It is accredited by the Community Health Accreditation Program (CHAP). Its Hospice Program is Medicare Certified and State of Connecticut licensed. Referrals are accepted from physicians, hospitals, skilled nursing facilities, or from patients
themselves. Community support from area towns, United Way of Western Connecticut and tax deductible gifts, bequest and memorial contributions help to fund many of NMVNA’s community programs.
